2020-'21 Waiver

BEFORE A CHILD CAN PARTICIPATE IN NORTH PARK HOCKEY ASSOCIATION, THIS RELEASE MUST BE SIGNED BY THE PARTICIPANT'S PARENT/GUARDIAN. SKATE AT YOUR OWN RISK. 

I acknowledge that participation in athletics carries with it a risk of personal injury. I agree that North Park Hockey Association, its agents, contractors and volunteers shall not be liable to me or my child for any injury, illness or damage, howsoever caused, resulting directly or indirectly from my child's participation in any and all North Park Hockey Association programming and/or any time preceding, during, or after any program and I hereby discharge North Park Hockey Association, its agents, contractors, and volunteers, from all actions, claims and demands I or my child may have for any such injury, illness or damage. I agree that I've used my child's legal name and the birthdate that is on his or her birth certificate.

Only players registered or wait-listed will be considered for roster spots. A completed registration does not guarantee a roster spot. In addition to practices, your child’s North Park team may participate in scrimmages, exhibition competitions and/or games with other hockey organizations. For all practices, scrimmages, exhibition competitions and/or games, you agree to cooperate with, and follow, all required protocols from various state regulations, local authorities, Department of Health guidelines, officially sanctioned USA Hockey affiliates and rink facilities.  

Given the state of COVID-19, your child’s participation in scrimmages, exhibition competitions and/or games is completely voluntary. If, on behalf of your child, you choose not to participate, your child’s place on his or her respective team will remain secure.     

Participation in an ice hockey program may create for your child a risk of exposure to various communicable diseases, including COVID-19. Participating in games with other hockey organizations may create a higher risk than that associated with an internal North Park practice.  

You acknowledge that by participating in practices, scrimmages, exhibition competitions and/or games as a member of North Park Hockey Association, Inc. you accept sole and exclusive responsibility for the risks of any associated health exposures for your child and your family. With such acknowledgment, you agree to indemnify and hold harmless the North Park Hockey Association, Inc., its agents, contractors and volunteers.
